1/2 day courseThis program will provide participants with the information, tools and methods to actively participate in the investigation process.

Anyone who believes that accidents happen simply as a result of carelessness, miss an opportunity of not seeing the true reason that it occurred and how to prevent it happening again.

This course is ideal for participants who have key Work, Health & Safety roles within the organisation.

Understand legal responsibilities, including:

Understand the key steps in an incident investigation, including:

  • Initial steps following an incident
  • The key steps
Element 3

Identify the key information that is required, including:

  • Who, What, When, Where, How and Why?
Element 4

Plan and conduct an investigation, including

  • No blame philosophy
  • Why it is important to investigate incidents
  • Who should investigate?
  • The strategy of an investigation
Element 5

Analyse the data gathered during the investigation, including:

  • Isolate the essential contributory factors
Element 6

Develop and evaluate corrective measures, including:

  • Risk assessment and control
  • Implementing corrective action

Prepare an incident investigation report, including:

  • What the report consists of
